  1. Passport:

The bearer’s signature or thumb impression should appear on page 2 of a Pakistani passport. If the applicant possesses previous passports, all of them should be submitted. In case of a lost previous passport, an FIR in English from the nearest police station should be attached.
  2. Photo:
